A sexual problem or sexual dysfunction can be defined as inability of a person to perform normal sexual activities or lead a good sexual life. These days sexual problems in male are common and according to a study 1 in 10 men experience sexual problems in their life.

Some of the very common sexual disorders in men are erectile dysfunction, premature ejaculation (also called quick or fast ejaculation), impotence, delayed or retarded ejaculation, no ejaculation or absent ejaculation, small or thin penis, venereal diseases, low sex desire (loss of libido), weakness after sex, sensitive glans, infertility, nocturnal emission, wet dreams, inability to consummate after marriage, sexual transmitted diseases (STD) etc. Many of these disorders are treatable and can be cured completely provided if you share your concerns to a sexologist or sexual doctor.

Erectile Dysfunction or ED can be defined as the inability to develop or maintain the erection that is required sexual intercourse. The other terminologies used for erectile dysfunction are impotence, erectile failure, sexual weakness, napumsaka, napunsakta etc. Sexual arousal in men is characterized by erected penis which is the symbol of man’s masculinity. Erected Penis represents manhood of the person. Erectile Dysfunction can be caused due to physical or psychological problems. The psychological problems include stress, depression, anxiety and mental disorders. The physical factors that causes E.D. are diabetes mellitus, drugs, stroke, epilepsy, smoking, aging, hormonal imbalance, high blood pressure, cardiovascular diseases etc.
Premature ejaculation is an uncontrolled ejaculation wherein the man expels his semen prior or immediately after vaginal penetration with minimal stimulation. Premature Ejaculation (PE) is one of the most commonly found sexual disorders in men. This is found in approximately in 30% of the men. This condition is also called fast ejaculation, quick ejaculation, sheegra patan (seegra patan), shighrapatan, rapid ejaculation, rapid climax etc. Premature Ejaculation is characterized by early ejaculation of the semen before the man experience complete satisfaction. A man with premature ejaculation finds himself guilty for not satisfying his sexual partner. Early ejaculation leads to less or truncated sexual pleasure and may bring dissatisfaction among the partners..

Absent Ejaculation on No ejaculation is a condition in which the man fails to ejaculate semen even after prolonged sexual activities. In this disorder man gets orgasm and erection as usual but fails to ejaculate. The psychogenic issues are common causes for absent ejaculation. Certain drugs and medications also can cause absent ejaculation.

Retrograde Ejaculation: This is the condition in which man gets normal orgasm and erection, but the semen is discharged to bladder. A simple urine test to check the presence of sperms can confirm the retrograde ejaculation.

Venereal Diseases (VD) or sexually transmitted diseases (STD) are the ones which gets transmitted from one person to another by means of sexual contacts. The sexual contacts can be vaginal intercourse, oral sex or anal intercourse. The common STDs are gonorrhoea, syphilis, genital warts, HIV, AIDS, Chlamydia, genital herpes etc. These venereal diseases spread through semen, vaginal fluid, breast milk, saliva etc. By practicing preventive sex or safe sex methods these sexually transmitted infections can be prevented. Sexually transmitted diseases can be treated effectively if detected at early stage. But some diseases have no treatment.
Spermatorrhoea is a condition in which man involuntarily releases his semen without the act of sexual intercourse or orgasm. In spermatorrhoea usually semen is released to urine during urination or defecation. It is commonly seen in unmarried persons.
If the penis is thinner than the average, the condition is called thin penis. Thin penis is observed in approximately two percent of the men worldwide. Having thinner penis may lead to low self esteem, sex phobia, unsure of sexual capability in men. The most common reason for thin penis is improper release of some sexual hormones when the boy is growing. In some cases thin penis disorder is associated by birth due to chromosomal abnormality. Sluggish growth of penile tissue also causes thin penis.

Whatever may be the reason, the proper diagnosis and treatment will help to increase the thickness of the penis. Usually by hormonal treatment thin penis disorder can be cured. In very rare cases surgical intervention is required for penile girth enhancement.

When the desire for sex is below average it is termed as low sex desire or low sex drive. According to a study, approximately 1 in 5 men suffer from low sex drive at some point of their life. The loss of libido leads to dissatisfaction in opposite partner. Sometimes even the men who are suffering from low libido get frustrated because of this. Age is one of the factors for low sex desire, but not always. Several health disorders like diabetes, high blood pressure, anxiety, stress, depression, obesity, drugs, medications and chronic diseases can cause low sex desire in men. Decrease in testosterone hormone level also causes low sex libido. Low sex drive occurs both in men and women. This can cause relationship issues between couples, hence the people who are suffering from low sex drive need to consult the sex therapists immediately.
It has been observed that many men feel weak, tensed, fatigued and tired after having sex or masturbation. This condition is called post ejaculatory asthenia. Some people even complain that they have lack of memory after indulging in sexual activities. This may be cause of worry in most of the men. Hormonal disorders are said to be most common reason for sexual weakness. Other reasons for weakness after sex are health disorders, drugs, medications, psychiatric diseases, excessive smoking, alcohol addiction, chronic diseases etc. Upon proper investigation and diagnosis, sexual weakness can be cured in couple of months.
The glans is the sensitive bulbous structure at the end of penis head. In some men this is very sensitive and causes pain when touched. Sensitivity like this is unusual. The hyper sensitive glans may pose problem during sexual intercourse or while cleansing the penis under foreskin. So this problem has to be addressed on priority.
Azoospermia (commonly referred to as “no sperm count”) is a male fertility issue many men face.Azoospermia is defined as the complete lack of sperm in the ejaculate. It occurs in 5% of infertile men. If this is the case, then one or both of two conditions may be present (“causes”):

  • There is a problem with sperm production.
  • There is a blockage such that sperm production, although normal, cannot reach the ejaculate

Causes for Azoospermia

Conditions that cause azoospermia are listed in Table 1. If at all possible, treating the specific condition that is causing the azoospermia may reverse the process and lead to sperm production. This is especially true for azoospermia due to hot tubs or hot baths or testosterone supplements. In other cases, such as genetic infertility, this is not possible and assisted reproduction offers the best solution to family building.

  • Y chromosome microdeletions
  • Genetic infertility due to abnormal chromosomes (karyotype)
  • Unexplained genetic infertility
  • Secondary testicular failure, Kallman syndrome
  • Unexplained gonadotropin deficiency
  • Hypothalamic/pituitary tumor
  • Hyperprolactinemia
  • Cancer treatment (chemotherapy, radiation, surgery)
  • Varicocele effect
  • Pituitary suppression, drug induced (anabolic steroids, alcohol, glucocorticoids)
  • Testosterone supplements
  • Congenital adrenal hyperplasia
  • Severe illness (cancer, kidney or liver failure)
  • Diabetes mellitus
  • Sickle cell anemia
  • Hemachromatosis
  • Sperm autoimmunity
  • Pesticide/toxin exposure (including hot tubs and baths)
  • Undescended testicles at birth
  • Obstruction, congenital absence of the vas deferens (CAVD)
  • Ejaculatory duct obstruction
  • Epididymitis
  • Scrotal trauma or surgery
  • Young syndrome
  • Vasectomy

Oligospermia – Low Sperm Count

Oligospermia is a male fertility issue defined as a low sperm concentration in the ejaculate. Low sperm concentration or “sperm count” is the number of sperm in a presc of ejaculate (typically milliliters or mL).

Nocturnal Emission or Wet Dream is an ejaculation of semen during the sleep. It is also called night fall. It is the spontaneous orgasm during sleep with ejaculation in male and vaginal wetness in females. This is most common during teenage and early adulthood. This is the natural way of expelling semen in men. Nocturnal emission is usually accompanied by erotic dreams. As said above, nocturnal emission is spontaneous process and it is more common when the person is sexually inactive; intercourse or masturbation. Wet dreams are very normal and do not require any medical attention since it is the natural way of expelling semen generated in the body. Once the person starts having sexual intercourse or doing masturbation, the frequency of nocturnal emission will reduce.
Some couple can’t perform sexual intercourse after marriage due to disorders like erectile dysfunction, premature ejaculation, vaginismus etc. The cause of problem could be from male or female. Non-consummation of marriage will cause relationship problem and may lead to annulment. Therefore it is necessary that root cause of the problem is identified during early days of marriage and solution is found to facilitate consummation.

Male infertility can be defined as incapability of a man to make his fertile wife pregnant even after one year of unprotected intercourse. Infertility affects 7% of the men. This is nearly around 40-50% of the total infertility observed in couples. A man’s fertility depends on quality and quantity of the sperms in his semen. A semen analysis is done to know whether the man has infertility problem which will reveal the quality and quantity of the sperms. Semen analysis test can detect whether the condition is Azoospermia (no sperms in the semen), Oligospermia (only few sperms in semen), Asthenozoospermia (reduced sperm motility).

The common causes for male infertility are sexually transmitted diseases, blockages in testicles, genetic diseases by birth, retrograde ejaculation, hormonal problems, erectile dysfunction, varicoceles, prostrate related problems, premature ejaculation, obesity, stress, consumption of drugs, excessive alcoholism, chronic smoking etc.

The delayed ejaculation or retarded ejaculation is a condition in which man takes very long time for semen discharge. The man wants to ejaculate, but he is unable to do so easily. The people who are suffering from this problem get normal erection, arousal but have problem in expelling semen in woman’s vagina. Some men who are suffering from this disorder can easily ejaculate through masturbation. Delayed ejaculation was believed to be very rare condition, but now a days this is on increasing trend. The reasons for retarded ejaculation are some psychological problems or hormonal problems. The delayed ejaculation may upset the partner and hence needs to be addressed at the initial stage itself.
